Injury Report Form

If a player has been injured, guardians must complete and submit the above form within 72 hours to info@ssunitedfc.com

*Parents/Guardians need to keep coaching staff updated on condition and or if player visits any of the below. 

• When Emergency  Service has been called 
• Report to ER/Hospital 
• Visited Medical Doctor 
• Physiotherapy/Rehab 

Coaching staff if injured and need medical attention please fill out the form and contact The Club.
